ARCUS WASHINGTON HEALTH DATA PRIVACY POLICY

This Washington Health Data Privacy Policy (’WA Health Data Privacy Policy’), applicable solely to Washington residents (‘WA resident’), explains:

• The categories of Health Data that we collect from WA residents on our Site – https://arcusbio.com and http://trials.arcusbio.com;
• The purpose(s) for which the categories of Health Data are collected and used;
• The categories of sources from which the Health Data is collected;
• The categories of Health Data that is shared;
• The categories of third parties with whom the Health Data is shared; and
• How WA residents can exercise their rights as provided under the Washington My Health My Data Act (‘MHMDA’).

This WA Health Data Privacy Policy supplements the Arcus Privacy Policy.

If you are a WA resident, you have a number of rights over your Health Data which may be subject to limitations and/or exceptions. These include the right to:

1. request access to your Health Data and obtain confirmation on whether we collect, share, or sell your Health Data;
2. withdraw your consent from our collection and sharing of your Health Data;
3. request deletion of your Heath Data.
